Medicare GUIDE Dementia Support & Respite Care

Additional support for families caring for someone with dementia

Caring for someone with dementia can be demanding. Medicare's GUIDE program may help eligible individuals and their unpaid caregivers access coordinated dementia support, caregiver education, community resources, and respite care.

Get Care at Home works with Medicare GUIDE organizations to help Sacramento-area families understand their options and take the next step. When permitted by the program, Get Care at Home may also provide the family's in-home respite care.

What is the Medicare GUIDE program?

GUIDE stands for Guiding an Improved Dementia Experience. It is a Medicare program designed to improve the quality of life for people living with dementia while supporting the family members and friends who help care for them.

Support that may be available

• Dementia-focused care navigation
• Virtual access to a dementia-trained care team
• Care coordination with current doctors and specialists
• Medication review
• Caregiver education and one-on-one support
• Referrals to community and clinical resources
• Medicare-funded respite care for qualifying families

Who may be eligible?

A GUIDE organization must make the final eligibility decision. In general, the person receiving support may need to:

• Have dementia or be completing the process needed to confirm a diagnosis
• Be enrolled in Original Medicare Parts A and B
• Live at home or in an assisted living setting
• Not be enrolled in Medicare Advantage
• Not be enrolled in PACE
• Not be receiving hospice care
• Not be a long-term nursing home resident

Additional requirements may apply for respite care, including the person's level of dementia and the involvement of an unpaid family or friend caregiver.

Respite care through Get Care at Home

Qualifying families may receive Medicare-funded in-home respite care. A trained caregiver can provide supervision, companionship, personal care support, meal preparation, light housekeeping, and relief for the unpaid caregiver.

The amount and availability of respite care depend on the GUIDE organization, the person's eligibility, and the current Medicare program year.

How the process works

1. Submit the short interest form below.
2. We contact you to review the basic information and answer questions.
3. We connect you with an appropriate GUIDE organization when the person appears eligible.
4. The GUIDE organization confirms eligibility and develops the care plan.
5. Get Care at Home may provide respite care when authorized under the program and partnership agreement.

Get Care at Home serves Citrus Heights, Roseville, Rocklin, Sacramento, Fair Oaks, Carmichael, Orangevale, Rancho Cordova, and Folsom.
